After all manner of mutation and transformation, it appears Tomura Shigaraki has achieved a fitting (but disturbing) 'final form' in My Hero Academia.
The biggest foe right now in My Hero Academia is undoubtedly Tomura Shigaraki, who recently proved how serious he was by actually bringing about the death of one of the young heroes battling against him. For the entire fight so far, however, Shigaraki's body has been in a state of flux, desperately seeking a form more suited to the power he now wields, and in chapter 366, it appears he's finally found it.
Despite the heroes' use of the Erasure quirk to limit Shigaraki's ability to activate his own quirks, the villain has proved a dangerous opponent due to the mutations he's been undergoing. A mass of mouths and fingers, Shigaraki looked more like something out of anime classic Akira than a supervillain for most of this fight, his physical form reflecting the absolute loss of humanity that's going on within.
In chapter 365, Shigaraki internally lamented how no one reached out to help him in his darkest moment, just after those family members had been accidentally slaughtered by his manifesting Decay quirk. All For One had encouraged him to keep their severed hands close to prevent him from ever having a chance at getting over it, and yet the "touch" of his family's hands seemed to provide him some degree of relief, at least until they were destroyed.
On a metafictional level, not only does this form call back to his original appearance, but it also reflects just how hung up on that moment his psyche is. The deaths of his family are his villain origin moment, and like any good villain, that origin has ultimately shaped his appearance to the very end.
