Paramedics were also left waiting in reception
Events leading to the death of a patient from brain damage included medication mistakes and paramedics left with no one to meet them. Michelle Louise Whitehead, who lived with her husband and children in the Rainworth area, was 45 years old when she died on May 7, 2021.
An ambulance took Mrs Whitehead to King's Mill Hospital where she was placed in the intensive care unit. "She suffered irreversible brain damage," said Assistant Coroner Laurinda Bower. The inquest into her death in Nottingham heard from one witness yesterday - Hannah Marsh, a lead nurse in charge of the shift on the ward at Millbrook before Mrs Whitehead died.
The nurse was not aware of the condition "psychogenic polydipsia" and she was not aware of any risks of taking on a volume of water in a short space of time. Assuming it was Haloperidol she was giving to Mrs Whitehead, she did not double check the syringe or the volume, but admitted she should have - which is a requirement. By now the nurse had written up a note to say Mrs Whitehead had the Diazepam and Haloperidol.
Nurse Marsh admitted if she had seen them, she would have challenged them. Additionally, the inquest heard Nurse Marsh had also taken a physical observation sheet away to write up as part of the documentation required. Miss Marsh went to Mrs Whitehead's room and she was unconscious. She had swollen lips and tongue. She was not put in the recovery position until Miss Marsh got there.
