Family still has questions following son's death after he contracted sepsis from dislodged feeding tube

The family of a man who died when his feeding tube became dislodged say they still have questions about the way he was cared for in the days before his death.

However Andrew's vital signs began to indicate he had an infection and staff called 999. Andrew was taken to A&E where he was treated with anti-biotics, and arrangements were made for a pegogram to take place - a scan to check the positioning of the peg feeding tube. Dr Thind, a consultant radiologist at Royal Preston Hospital, said a review of the pegogram procedures had taken place following Andrews death. She said: "We decided in hindsight that we should change the process of how we identify whether the tubes are in the right place." She said it was likely the tube was dislodged at the time of the first test on May 23, however it had not been detected.

Andrew died in hospital at around 10.30pm on May 26, surrounded by his family. Area coroner Chris Long said there was a delay of two to three days in discovering the tube had been displaced. The coroner recorded a narrative conclusion, saying Andrew's death was as a result of sepsis, a perforated abdomen after dislodgement of the feeding tube, and a hypoxic brain injury post cardiac arrest as a result of the illicit use of opiates.
